It is strongly encouraged that contact be made two months prior to the start of the semester in which the student plans to enroll. Some accommodations may require more time to arrange.
Early contact will ensure that accommodations are available for the admission process and when classes begin.
Late contact by the student could delay the availability of accommodations in time to begin classes.
Accommodations cannot be authorized without appropriate documentation. It is the student's responsibility to provide the Disability Services Office with recent documentation from a licensed professional (including school psychologists) verifying the need for the accommodation requested. Services and accommodations are individualized but must be provided on a consistent basis according to these and related established procedures.
Individuals with disabilities are also encouraged to contact their local Office of Vocational Rehabilitation to learn if they qualify for the many benefits available through that agency.
Mission Statement
The mission of Disability Support Services (DSS) is to provide access to College programs, services, activities, and facilities for students with disabilities; to provide student advocacy; to encourage the highest levels of academic and personal achievement; and to advocate for an accessible environment for faculty, staff, and visitors
In order to better serve you please print out the form(s) that you need for each semester that you will be taking classes at Ivy-Tech Community College. These forms are valid for face to face classes or internet class.
Please remember that it is the responsibility of the student to request services and / or to continue to receive services. If you do not complete these forms you run the risk of not having your accommodations in place for the future semester you are planning to attend.
Forms will also be available at your local D.S.S. office. Please turn in your forms when you register for classes.
