In compliance with Section 504 of the Rehabilitation Act of 1973 and Title II of the Americans with Disabilities Act (ADA), Ivy Tech Community College of Indiana assures protection from discrimination and provides auxiliary aids and services to qualified students in all academic programs and college activities.
In order to be protected by Section 504 of the 1973 Rehabilitation Act and Title II of the ADA, a student must identify himself or herself as being a person with a disability by providing documentation.
Acceptable documentation will describe the current impact of the disability as it relates to the request and be prepared by an appropriately credentialed individual who is not a family member. It will be typed and printed on the letterhead of either the practitioner or the agency providing the assessment, dated, signed and otherwise legible. Encouraged within the documentation are any suggestions of auxiliary aids and services that might be appropriate at the postsecondary level.
Students requesting auxiliary aids and services must fill out a Disability Accommodations Request Form prior to each semester. It is the student's responsibility to contact the campus Disability Support Services representative to request accommodations; any information shared will be kept confidential unless that student authorizes release and exchange of specified information. Requests for accommodations and documentation of disability must be received one month prior to enrollment for the next academic term. Additional time may be required for some requests. Every effort will be made to provide reasonable accommodations in a timely manner.
The Disability Support Services coordinators have prepared a booklet entitled "AWEDS: A Guide for Students, Faculty, and Staff." The booklet defines disabilities, has a question and answer section, and then describes the various types of possible accommodations.
